The Dublin City PPN SPC Housing Linkage Group would like to invite all PPN members to a meeting on 15 July from 6.00pm – 7.00pm on Zoom.
Eimear McCormack will make a presentation and take questions from the group on 15 July between 6.00pm – 7.00pm on Zoom. Eimear is the coordinator of the Healthy Age Friendly Homes Programme, which aims to support older people to remain living at home, in their community, rather than going into residential care, and Eimear will be available to talk about this programme on Thursday 15 July.
The Healthy Age Friendly Homes Programme aims to support older people to live in their own home with dignity and independence, for as long as possible, and reduce the number and rate of transfer to long-term residential care. The programme’s ambition is to enable older people to live longer, healthier lives by keeping care closer to home and expanding the range of health and social care services in the community. The project is a partnership between Age Friendly Ireland, a shared service centre operating the national age friendly programme, and Slaintecare, the government’s ten year programme for reform of the health care sector. This project represents an innovative approach in operating between housing services in local government and health.
